After School Sports Program

1. Refund Policy: No refunds will be issued after the first week of practice. If you decide to withdraw

your child from the program, please notify us in writing prior to the end of the first week of practice to

be eligible for a refund. After the first week, all fees are non-refundable regardless of attendance or

participation.

2. Weather Cancellations: Practices may be canceled due to inclement weather, including rain and

thunder. While we will make every effort to reschedule canceled sessions, we cannot guarantee that

all sessions will be made up. Notifications of cancellations will be communicated via text or email.

3. Media Consent: By enrolling your child in the program, you grant permission for your child's

image to be used in promotional materials, including social media posts and other online platforms.

This helps us showcase the program and celebrate the participants' achievements.

4. Communication Consent: You agree to receive communications related to the program via text

and email. This includes but is not limited to, schedule updates, practice cancellations, and other

important announcements.

As the parent/guardian of the above-named child, I acknowledge that participation in sports involves inherent risks, including the risk of injury or death. I hereby release, discharge, and hold harmless the after-school sports program, its coaches, staff, and volunteers as well as Chabad of SW Broward, it's staff and volunteers, from any and all claims arising from my child's participation in the program. I also agree to allow my child to receive emergency medical treatment if necessary.  My submission of this form shall count as my consent to the above.
